TCPLogView

TCPLogView is a simple utility by NirSoft that monitors opened TCP connections on your system and logs each connection's open and close events with details such as event time, event type, local and remote addresses and ports, process ID and name, and remote IP country information (requires additional file). It runs on Windows 2000 through 8 and is portable with no dependencies.
