Submit Forms to the Board
Use the following reports and forms to submit directly to the cwACT Board. In order to be considered and discussed at the monthly Board Meeting forms need to be submitted no later than the Friday of the week prior to the meeting.
Production Report
Use this form to provide updates to the board on the status of an active production. Communicate progress, ticket sales, concerns, and any support or resources needed ahead of monthly board meetings.
Committee Report
Use this form to submit reports to the board ahead of monthly meetings. This form collects updates on recent progress, changes since the last report, and any support or discussion items the committee needs from the board.
Proposal Form
Use this form to submit a proposal to the cwACT Board. Want to work with cwACT, or have cwACT involved in your projects in some way? Fill out this form and it will be considered at the next board meeting.
