Inspiration

CareLoop was inspired by real gaps in healthcare follow-ups, especially in rural and high-pressure settings. Missed appointments, weather disruptions, and poor communication often leave referrals unresolved. We wanted to build a system where that simply can’t happen.

What it does

CareLoop is a comprehensive referral management platform that ensures no patient falls through the cracks. It connects nurses, specialists, and patients through three dedicated portals, tracking every referral from creation to completion. The system automatically prioritizes appointments based on urgency, sends timely notifications via SMS and email, monitors weather conditions that could affect travel, and flags missed appointments for immediate follow-up. When patients miss visits or referrals stall, CareLoop escalates to care teams with clear action steps. It works offline-first, so nurses can manage referrals even without internet, syncing automatically when connectivity returns. Every stakeholder sees exactly what they need. Nurses manage workflows, specialists handle their caseload, and patients receive clear guidance. All in one unified system that guarantees accountability.

How we built it

We built CareLoop as a modern web application with a state-driven backend at its core. The system tracks referral status, ranks appointments by priority, detects missed visits, supports offline first workflows, and sends automated notifications to patients and care teams to close the loop at every step.

Challenges we ran into

One major challenge was designing workflows that are both powerful and simple enough for nurses to use under time pressure. Handling edge cases like missed appointments, proactive rescheduling, and escalation logic, while supporting unreliable connectivity required careful planning and iteration.

Accomplishments that we're proud of

We’re proud of creating a system where every referral has a guaranteed outcome. The clear lifecycle model, automated follow-ups, and escalation safeguards ensure accountability and reduce the risk of patients falling through the cracks.

What we learned

We learned how critical reliability, clarity, and human-centered design are in healthcare software. Building CareLoop reinforced the importance of state-driven systems, thoughtful notifications, and designing for real-world constraints, not ideal conditions.

What's next for CareLoop

Next, we plan to expand analytics and reporting, improve decision-support for nurses, and deepen integrations with existing healthcare systems. Our goal is to make CareLoop scalable, deployable, and impactful in real clinical environments.
