🔥 Only 16 COMPLETE teams can participate—first come, first serve!
💵 Player Fee: $50 per player
  $20: Registration Fee
  $30: Team Shirts
🧍‍♂️ Team Capacity: 7–10 Players | Age Requirement: 16-40 | This is a co-ed tournament. All genders are eligible to play.

🏆 1st, 2nd, and 3rd place trophies will be awarded
🏀 This will be a single-elimination tournament, so teams are not guaranteed multiple games.

IMPORTANT:

📌 All players must be members of the church they’re representing.

📌 Your church must be aware of and approve your participation.

📌 Only TEAM CAPTAINS should complete this form.

📋 A complete roster and fee confirmation IS REQUIRED to play.

📍In the Notes section, please include the name of your church and the number of participants you are registering to play

🚫 Incomplete or individual payments will not be accepted. Teams with unpaid or incomplete registrations will NOT be allowed to participate. Payments are due June 27th

📄 Once this Google Form is completed, a CBT Mass committee team member will reach out to you with the player contracts.
