My child will bring their cellphone and understands that it should not be used unless making an emergency call or contacting a parent. Pictures will not be taken by my child on their cellphone.

Signature (By entering your name), you are agreeing to drop your child off no earlier than 4:45pm and to pick them up no later than 8:15pm. You are authorizing EFBC to feed your children. You are also authorizing EFBC to seek medical treatment for the child(ren) you leave in our care in the event of an emergency. Further, you agree to hold EFBC faultless for any accidents or injuries the child(ren) may sustain in our care. We love children and will do our absolute best to care for them! However, there are risks associated with some of the activities including sprains, cuts, and bruises, among other possible injuries, and we want you to be aware that by signing you are agreeing to hold EFBC faultless for any accidents. I also give EFBC permission to use my child's likeness in social media advertising and understand that photos and videos will be taken all week.
