Cabin Aviation Safety Action Program is designed for Flight Attendants to report and the Company to identify and correct specific safety issues.

Cabin Cleanliness

Report the level cleanliness on the aircraft including the lavatory, galley, cabin, and Flight Attendants work area

CERS Report

Submit a CERS Report if you encounter any event or hazard that needs the company’s attention, emergency landing, turbulence, medical emergency, or other safety issues.

Crew Scheduling Feedback

Submit your experiences with Crew Schedule, Crew Tracking, Crew Scheduling Systems and the Hotel Limo desk

CQ Web-Based Training (WBT) Module Length

Report and document the actual time it took you to complete the CQ web-based training

Early Boarding Violation

Report an early boarding violation to help APFA compile and track violations

Hot Cabin Report

Report hot cabin temperatures and any symptoms felt by passengers and crew members

Hotel Delay Compensation Form

This form is only for compensation hotel delays if you experience a hotel delay and issues with receiving compensation

Hotel / Transportation Feedback

Report negative or positive experiences with hotels and transportation to and from the airport

IROPs Report

Report long wait times with Crew Scheduling, no hotel rooms/transportation, illegal rescheduling, Missed Trips and Late Reports due to IROPs

Jumpseat Denial Report Form

Report if you’ve been denied a cabin jumpseat on a mainline flight due to a weight restriction and the standby list is cleared out of order

NOD Filing Request Form

Submit details for a potential Notice of Dispute (NOD)

Sedgwick Lack of Contact (IOD)

If you have reached out to Sedgwick with an IOD claim and it has been more than 72 hour or 3 business days without contact file a report here

Sit Rig Issue Report Form

Report if you have a Sit Rig Issue such as incorrect amount, missing pay, reserve Sit RIG not paid above Reserve Guarantee, or another issue

Report a Smoke, Odor, or Fume (SOF) event on an aircraft
