What does it mean to be rostered with a blue sky physician?
Our physicians and the Blue Sky staff work together to provide a high standard of interprofessional primary care. We use an Electronic Medical Record (EMR) to coordinate your health information and organize our schedules to offer same day appointments and After Hours care, as well as appointments with our team of Internal Healthcare Providers.
What Is A Family Health Team (FHT)?
Family Health Teams are health care organizations staffed by a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide health care for their community.
The function of a Family Health Team is to provide a wide range of services and care options to rostered patients. Family Health Teams ensure that people receive the care they need in their communities, as each team is set up based on local needs. They focus on chronic disease management, disease prevention, and health promotion, and work with other community-based health care organizations such as the Health Unit and Ontario Health at Home.
Looking for a family health care provider or need non-urgent health advice via telephone?
Health811 is designed to help you find a family health care provider. You may want to consider registering for the program if:
- You are actively looking for a regular provider for ongoing family health care needs;
- You have a valid OHIP card; and
- You are not currently enrolled with a family health care provider according to Ministry of Health.
To register with the program, simply call 1-866-797-0007. You will need a pen, some paper, and your OHIP card when you call Health811.
Health811 is also available for patients to access non-urgent health advice from registered nurses 24/7 via telephone (811) or online chat at ontario.ca/HealthConnectOntario.
