Employees
- Employee ADA Documentation of Disability Form (Provider)
- This form is for employees requesting ADA Accommodations through the University Compliance Office (UCO). Employees with an obvious, chronic disability are not required to submit this form.Student employees who are requesting accommodations for both academics and on-campus employment should speak with their Accommodations Specialist before submitting this documentation

- Voluntary Self-Identification of Disability (Employee)
- We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years. Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. Students
- Provider Documentation Form (Students)
- Only licensed health care professionals may submit this form to share documentation with UCO. Documentation must confirm the fact that the disability is present and limits a major life activity or major bodily function with impacts on functioning.
- Non-Standard Test Scheduling Request Form
- The Non-Standard Scheduling Request Form (NSS Form) should be used if you have missed the one-week deadline to schedule your exam through the Hub.
- Consent for Release of Information from Student Education Records
- Students may complete this form to request for UCO to share information such as an Accommodation Plan or documentation to a specific person or agency. You should complete this form if you would like a copy of your accommodations or documentation and check “Other” and enter “Release to Self”, or if you need us to send information to Study Abroad, another institution, or to a standardized test agency (GRE, MCAT, LSAT), etc.
- Genio Loan Request Form
- If you have access to Genio as one of your approved accommodations, please fill out this form to request an account and an UCO staff member will contact you with access information.
- Deadline Extension Request Form
- The Limited Per-Assignment Deadline Extension Request Form should only be used if you have this accommodation and have already discussed your accommodation plan with the instructor. Use this form to request a per assignment deadline extension due to a disability.
- Alternative Format / E-Text Request Form
- At least six weeks prior to the start of the semester, students must contact their instructors to discuss any alternative text needs for their courses. Students must work with instructors to identify any required textbooks, in-class handouts, Sakai/Canvas or other materials posted in a classroom management system, and any other required readings that will need to be made available in an accessible format. Once students have identified the materials needed to be converted, they should submit an alternative format request to UCO.
- CART Request Form
- UCO uses an external agency to provide CART (Communication Access Real-time Translation) Services to ensure equal access and reasonable accommodation in classes, services, programs and activities for students who are deaf or hard of hearing. Requests should be submitted at least 4 weeks in advance to ensure CART Services can be arranged.
- ASL Request Form
- If you receive ASL interpreting as one of your approved accommodations, please complete this form to request interpreting services for any course or other commitment. Please submit requests as early as possible, as providing advance notice helps ensure interpreter availability and allows time for coordination.
The University is required to collect employee disability, ethnicity/race, gender and veteran status information to comply with state and federal laws. These forms are used to collect this information. The information below should be included in the new hire package that is sent by the hiring department or HR representative to all new hires, including temporary employees. If there is no new hire package, this information should be sent to the employee via email at the start of the position.

Skills assessment approval request forms must be submitted if any portion of the interview process involves assessing a candidate’s skills through the requirement of a presentation, case study or other tests assessing candidate skills. Submission of this request form is not required for standard interview questions, including situational or hypothetical interview questions. Submissions are reviewed weekly, but please submit this form at least two weeks prior to the date by which you are inviting candidates for interviews.

- Attorney and Non-Attorney Advocate Representation Form
- As used in this document, the term “Advocate” means a person who may accompany a party to investigative, administrative, or adjudicative meetings or proceedings under UCO policies and may fully participate in the proceeding to the same extent afforded to the represented party. An Advocate may be an attorney but is not required to be. For an Advocate to participate in an UCO meeting or proceeding, the party must complete and submit this form and the applicable Consent for Release of Information form.

- This form is used by students to give the UCO office consent for the release of information from Student Education Records.
- Employee Consent for Release of Information
- This form is used by employees to give the UCO office consent for the release of employees confidential personnel file information.
