The event must be licensed by COSCAP and the license fee must be paid by the organizer/promoter of the event or by the owner of the establishment. 2. The COSCAP "Live Performance Reporting Sheet" form must be completed in full and returned to COSCAP no later than (3) months after the event, otherwise the event will not be eligibe for payment 3.Evidence must be attached to confirm that the event took place. The Evidence required before a "Live Performance Reporting Sheet" form can be processed is outline in Section 4 below. SECTION 4 - For Performance in Concerts, Clubs, Bars and Similar Establishments Select any one of the following and attach relevant evidence Copy of complete performer contract Ticket/cover charge stub indicating the admission charge, date, performer and premises of the performance Copy of newspaper advertisement or poster, newspaper review, indicating the admission charge, date, performer and premises of the performance Attach Relevant Evidence Selected * Evidence must be attached, you can take a photo of any of the above (that you have selected) and attach it, also an email etc.Files must be less than 3 MB.Allowed file types: jpg jpeg png pdf doc docx odt. SECTION 5 - IMPORTANT All compositions performed must be listed by the correct title and have COMPLETE COMPOSER/AUTHOR AND PUBLISHER INFORMATION (if known). For Compositions other than your own, please make every effort to provide composer's names. If you perform a set list over a period of time at a particular venue, then you need to complete only one form stating the various dates of your appearances. However, each time the set changes a different form must be completed. A new one must also be completed for each venue. Return this form immediately following the gig or where you work at a hotel or nightclub, for example as soon as possible after the set has been decided. Members are also reminded to check COSCAP in advance of their performances to ensure that the venue is licensed. If a venue is not licensed then royalties are not collected. ALL FORMS MUST BE SIGNED BY THE PROMOTER/MANAGER/LANDLORD REPORTS ARE SUBJECT TO VERIFICATION BY COSCAP
