Community Health Needs Assessment Interest Form
Puyallup Tribal Health Authority (PTHA) is partnering with Seven Directions, a Center for Indigenous Public Health at the University of Washington, to conduct a Community Health Needs Assessment to understand the health needs, strengths, and priorities of our people. We are recruiting Puyallup Tribal Members and other Native folks living in Pierce and surrounding counties to participate in group listening sessions or to take a brief (15-min) online survey. Listening sessions provide a space for open group discussion where community members can share experiences and priorities, while the survey offers a quick way to contribute individual perspectives. Both activities are designed to gather valuable input that will guide improvements to PTHA services for community health and wellbeing. Please indicate your interest in these data-collection activities.
