Please download out this form to fill out and send to us for performances.

Technical Rider

For all student claims, please fill in the student claim form and submit to your respective staff advisor.

Please note that you will need to include both the receipts and a screenshot of the bank transaction from your banking app.

After submission, you must keep the receipts for at least a month.

For all non-SIT students and alumni participating in SIT activities, they are required to sign the indemnity form prior to the event. 

Please submit all signed forms to your respective staff advisor.

Training Logsheet

Before a self-organized event, the club is required to submit the full event proposal at least 1 month before the event date to your respective staff advisor.

A full event proposal consists of the following:
- OSE-GOV-FM-005 Event Proposal
- OSE-GOV-FM-005A Event Budget
- OSE-GOV-FM-011 Emergency Management Plan (EMP)
- Risk Assessment Management (RAMs)

After the event, the club will need to submit the following within 1 week:
- OSE-GOV-FM-006 Post Event Summary
- Upload all event photos and videos to the Media SharePoint folder.
