LAMP reporting and stats

All you people who run LAMP type servers, what stats packages do you run and can suggest?

The kinds of things I'm interested in are:

Must haves
General stats, what files have been hit and how many times.
Referrering pages and requestor stats.

Nice to haves
(near) Real time reporting of what files are being hit (this could be an additional console app)
Ability to integrate selected stats into outgoing PHP pages
