Survey tool

I'm considering replacing a home-made survey tool with a commercial or open-source alternative.

I need a robust instrument, one that allows logical sub-options (if answer to q4==yes, then show q6 else show q8). Also, I need very strong reporting capabilities, and the ability to export to PDF.

Suggestions?
