ok well i dont understand this

i have this code and i name it testform.php and i get this error
www.cs.ucr.edu/~yunc/testform.php

and i have the same code and i name it testform.html and i get it half way working
but it doesn't send
www.cs.ucr.edu/~yunc/testform.html

im pretty sure the code is correct because someone ran it on their webpage

ok im am really confused and no one knows what is going on
