    I certify that this application was completed by me, and that all entries on it and information in it provided by me are true and complete, to the best of my knowledge. I understand that any misstatements, omissions, falsifications, misrepresentations, or omissions of fact in this application may disqualify me from consideration for employment, or if I am hired, may result in disciplinary action up to and including termination of employment.

    I authorize PSS-College Bound, in order to evaluate me for employment purposes, to contact any and all of my current and previous employers, references, state or local law enforcement agencies and educational institutions, and otherwise to fully investigate my suitability for employment, character, general reputation, personal characteristics, mode of living, work habits, skills, and/or abilities. I understand that the results of any such investigation may be disclosed to personnel of PSS-College Bound and schools involved in the employment decision, and I consent to such disclosure.

    I understand and agree that PSS-College Bound is required by Texas Education Code and Board Policy to review my criminal history record information and that I may be required to send at my expense, or the Partnering District shall send to DPS at my expense, information that DPS requires for obtaining National Criminal History Record Information. Furthermore, I understand and agree that after the required information is submitted, if I am hired, I may begin employment, but that my employment is conditional upon review of my criminal history record investigation, and that I must be terminated at any time, if a determination is made that I am ineligible for employment.

    In connection with and in consideration of the PSS-College Bound's review of this application for employment and consideration of me for hire, I release and hold harmless from all liability of any kind, any PSS-College Bound member and its employees agents, trustees, and any other individual or entity that requests or supplies information in connection with any pre-employment background check, including liability resulting from negligence, gross negligence or any other cause, that I may sustain in connection with any pre-employment background check.

    If selected, I agree to accept the assignment to the work location and position as made by PSS-College Bound, and I understand that I may be transferred or reassigned at PSS-College Bound's discretion. I understand that this application and all supporting documents become the property of the PSS-College Bound, and no part of it is returnable or available to me.

IT SERVES TO NOTIFY ME THAT IF THE DISTRICT PERFORMS A CCH VERIFICATION CHECK BY ACCESSING THE TEXAS DEPARTMENT OF PUBLIC SAFETY SECURE WEBSITE, AND THE CHECK IS BASED ON NAME AND DATE OF BIRTH IDENTIFIERS THAT I SUPPLY, THE DISTRICT CANNOT DISCUSS THE RESULTS OF THE "NAME BASED CHECK" WITH ME, AND THE DISTRICT MAY REQUEST THAT I HAVE A FINGERPRINT SEARCH PERFORMED AT MY EXPENSE:
    DPS Computerized Criminal History (CCH) Verification

    I acknowledge that a Computerized Criminal History (CCH) check will be performed by accessing the Texas Department of Public Safety Secure Website and will be based on name and DOB identifiers I supply. (This is not a consent form.) Authority for this agency to access an individual's criminal history data may be found in Texas Government Code 411; Subchapter F. Name-based information is not an exact search and only fingerprint record searches represent true identification to criminal history, therefore the organization conducting the criminal history check is not allowed to discuss with me any criminal history record information obtained using this method. The agency may request that I have a fingerprint search performed to clear any misidentification based on the result of the name and DOB search. Once this process is completed the information on my fingerprint criminal history record may be discussed with me. In order to complete the process I must make an appointment with the Fingerprint Applicant Services of Texas (FAST) as instructed online at www.txdps.state.tx.us /Crime Records/Review of Personal Criminal History or by calling the DPS Program Vendor at 1-888-467-2080, submit a full and complete set of fingerprints, request a copy be sent to the agency listed below, and pay a fee of $24.95 to the fingerprinting services company.
