2026 Journey by Upstream Student CSM Sweepstakes Terms and Conditions

Eligibility: Open to U.S. residents aged 18+ who are currently enrolled in a CAPTE-accredited (or pending) Doctor of Physical Therapy program in the U.S. Employees of Upstream Rehabilitation, its affiliates, and their immediate family or household members are not eligible. Void where prohibited or restricted by law.

Giveaway Period: October 27, 2025 – December 1, 2025.

How to Enter: Visit https://app7.greenhouse.io/e/vdyhfq complete the entry form, and submit. One entry per person. Automated or multiple entries will be disqualified. No purchase necessary to enter or win; a purchase will not increase your chances of winning.

Prize: One winner will receive round-trip economy airfare for one from a major U.S. airport, lodging for 3 nights (Check in: February 12, Check out: February 15), and registration for CSM 2026 in Anaheim, CA, February 12-14, 2026 (Approximate Retail Value: $2,255. Prize is non-transferable, non-refundable, and cannot be exchanged for cash. Travel must occur on dates specified by Upstream. Winner is responsible for all incidental expenses (e.g., meals, transportation) and all federal, state, and local taxes. Upstream will issue a Form 1099-MISC for prizes valued at $600 or more.

Winner Selection and Notification: One winner will be selected at random on or about December 1, 2025. The winner will be notified via email and must respond within 7 days to claim the prize, providing proof of eligibility (e.g., enrollment verification). If the winner does not respond, a new winner may be chosen. The winner will be announced on or about December 2, 2025, via urpt.com and/or Upstream’s social media channels.

State-Specific Considerations: The Sweepstakes complies with all applicable laws and is not prohibited in any U.S. state. However, in the following states, additional requirements apply and will be strictly followed by the Upstream:

Arizona (AZ): A list of winners will be provided to the state within ten (10) days after the prize is awarded.

Tennessee (TN): Winners will not be required to grant publicity rights in perpetuity.

General Terms: By entering, participants agree to these rules and release Upstream Rehabilitation, its affiliates, and [social media platforms, e.g., Instagram, LinkedIn, Facebook] from any liability related to the contest or prize. Upstream reserves the right to modify, suspend, or cancel the contest due to unforeseen circumstances without obligation to award prizes. Odds of winning depend on the number of eligible entries received. Participants consent to the use of their name, likeness, and entry for promotional purposes without additional compensation, except where prohibited by law. Upstream Rehabilitation APTA CSM Grand Prize Giveaway

Official Summary Rules

NO PURCHASE OR PAYMENT NECESSARY TO ENTER OR WIN.

Sponsor:
Upstream Rehabilitation (“Sponsor”).

Eligibility:
Open to legal residents of the United States who are 18 years of age or older and registered attendees of the APTA Combined Sections Meeting (“CSM”) at the time of entry. Void where prohibited by law.

Contest Period:
The giveaway begins when the Upstream Rehabilitation booth opens at APTA CSM and ends at booth close on the final day of the conference.

How to Enter:
Participants will receive one (1) entry when their APTA CSM attendee badge is scanned by an authorized Upstream recruiter at the Upstream Rehabilitation booth. There is no cost to enter.

Bonus Entries:
Participants may earn additional entries by participating in the PuttSkee game at the Upstream Rehabilitation booth. Bonus entries are awarded at the time of participation. No limit on bonus entries during the Contest Period.

Prize Description:
One (1) Grand Prize winner will receive:

• One (1) $500 airline gift card
• One (1) $500 Airbnb gift card
• One (1) $500 Visa gift card
• One (1) luggage set (value TBD)

Approximate Retail Value (“ARV”): $1,500 plus the value of the luggage set.

Prizes are non-transferable and no substitution or cash equivalent is permitted except at Sponsor’s sole discretion.

Winner Selection and Notification:
One (1) winner will be selected in a random drawing from all eligible entries received during the Contest Period. The winner will be notified using the contact information associated with their badge scan. If the selected winner cannot be contacted or does not respond within a reasonable timeframe, Sponsor reserves the right to select an alternate winner.

Odds of Winning:
Odds of winning depend on the total number of eligible entries received.

Taxes:
The winner is solely responsible for all applicable federal, state, and local taxes and reporting requirements associated with acceptance of the prize.

General Conditions:
Sponsor reserves the right to cancel, suspend, or modify the giveaway if fraud, technical failures, or any other factor beyond Sponsor’s reasonable control impairs the integrity of the giveaway. Sponsor’s decisions are final and binding in all matters related to the giveaway.

Limitation of Liability:
By participating, entrants agree to release and hold harmless Sponsor and its affiliates from any liability arising from participation in the giveaway or acceptance or use of any prize.
