MySQL + PHP book - recommendations?

I'm sorry if this topic has come up in this community a hundred times before, I've just joined and I'm looking for your expert advice. If somebody has a very favorite book for a novice to learn the MySQL+PHP skill set, could you please share the info here? I do have some very basic knowledge on both topics, what I'm looking for is a "lesson-by-lesson" or "step-by-step" type of book that would help me to have a complete picture in mind on how to use them for my webdev projects correctly. I know there's tons of info online, but I do better with the tutorial/exersise type of learning.
