Could you help a sista out?

I am having a problem. I am a novice web developer and I have developed a page for my business using Macromedia: Dreamweaver MX. So far so good, but now I need to create a user authentication recordset and I am having problems setting up the table in MySQL. I use amhosting as my webhost and they are way too technical to help me out with anything. They get annoyed at the fact that I am so novice and do not understand everything they tell me. So maybe I will have more luck here? Here are the fields in the table setup for MySql, and I just don't know what to put. (i am setting this table up for passwords)
Field, Type, length/values, attributes, null, default, extra; and then i have a choice of Primary, Index, Unique, ---, or Fulltext.
I can't seem to get the right combination. I always end up with an error and no table.
If anybody is familiar with this, please help!
