Surveys (posted elsewhere too)

Okay, I've never done this before at all, so I really need help on it (although I bet it's easy, I just am a bit dense). I need to have a little survey included on a non-profit web site I'm making. I'm using (being forced to use) FrontPage, but can probably sneak in some Dreamweaver work. All it is is a Yes/No survey question, then I suppose, after the person answers it, they go to a page that tells them the percents so far of who answered yes or no so far. How on earth do I do this? Please please help!
