One of the biggest challenges in mobile aged care is what happens before the first session. Getting the right information — client preferences, goals, medical history, home hazard considerations, manual handling requirements — from care partners, in a consistent format, before a therapist walks through someone's door. When that information is incomplete or arrives piecemeal, everyone loses time. More importantly, the client misses out on care that is genuinely tailored to them from the outset. About six to twelve months ago I set out to fix that. Working within splose, we built a comprehensive referral form with conditional logic — customised fields that capture exactly what we need from care partners at the point of referral. Embedded directly into our intake process and linked from our website. It sounds like a small thing. It isn't. Every referral that comes into MTT now follows a clear pathway. It starts with that form. From there, Elise, our Service Coordinator and Clinical Lead, conducts a phone intake before any session is booked. By the time a therapist arrives at someone's door, they already know who they are walking in to see. That is not an accident. It is the result of having the right systems in place. That form has quietly become one of the most important pieces of infrastructure in our network. It means our therapists arrive informed. It means our care partners have a clear, consistent pathway to refer. And it means the client is already known before anyone knocks on their door. Splose didn't just give us a scheduling platform. They gave us the tools to build something that genuinely reflects the standard of care we are trying to deliver.
