What do these grades mean?
A
Excellent. Top marks — the latest inspection found no serious issues, or few enough to earn the health department's best band.
B
Good. A solid result with some violations on record — more than the top band allows, usually corrected on site or at a follow-up.
C
Needs work. The lowest passing band — enough violations to signal real problems, typically with a follow-up inspection required.
Passed. Met health requirements at the latest inspection. Some health departments report pass/fail instead of letter grades.
!
Conditional. Passed with strings attached — violations were found that must be corrected, often with a re-inspection scheduled. Not a failure, but not a clean pass.
Failed. Did not meet health requirements at the latest inspection. Many failures are fixed and re-inspected within days — check the inspection date.
!
Closed. The health department closed the establishment as of its latest inspection — usually for serious or repeated violations.
Not graded. The source hasn't reported a grade or result for the latest record. CleanPlate never invents one.

Grading systems vary by health department — some issue letters, others pass/fail. CleanPlate shows each city’s system as reported and labels anything it derives.
