A complete application consists of:

1. A signed letter of nomination or self-nomination, addressing the following points:
   * Name of individual or group of individuals under consideration for the award and their institution(s),
   * Name(s) and contact detail(s) (email required) of the nominator(s),
   * Description of the event/initiative, where in the case of an ongoing series of events and/or project, the description should address only activities from the past three years (maximum 2 pages),
   * Description of the impact the contribution has had towards making the mathematics community more equitable, diverse, or inclusive (maximum 2 pages), and
   * Description of some key challenges that had to be overcome in carrying out the contribution (maximum 1 page).
2. A minimum of two letters of support. For example, feedback from participant(s) regarding the impact the event/initiative had on them and/or providing perspective from faculty or staff on how the event/initiative contributed to the targeted community.
3. Other supporting materials (optional) e.g., websites, event publicity, photos/videos, testimonials, or impact data such as enrollment numbers, but please keep submissions brief and easy for reviewers to navigate (as a general guideline, something a reviewer could comfortably go through in one sitting, roughly around 20 minutes).

In the event that the nomination is not selected in the current award cycle, it will be retained for consideration in the following competition cycle (unless explicitly withdrawn).
