What is Medicaid?


Medicaid is a vital health insurance program that provides medical coverage, including long term services and supports, to approximately 21% of Pennsylvanians. That 21% includes people with disabilities, older adults, family caregivers and their children, direct care workers, low-income individuals and families and more. It is a joint federal and state program, meaning that both the federal government and individual states contribute funding and administration. How Does that Work?


It is called the Federal Medical Assistance Percentage (FMAP): the percentage of Medicaid expenses that the federal government pays each state. The current FMAP for Pennsylvania is roughly 53%. It is determined by a state’s per capita income relative to the national average.


What is Per Capita Income?

 

Per Capita Income is a measure of the average income earned per person in a specific geographic area. So, the federal government uses per capita income to determine how much financial assistance states need for their Medicaid programs.


FMAP is a reimbursement, so states must spend their own money first to receive the federal match. It is a key mechanism for distributing federal funds to states for Medicaid. It is designed to help equalize healthcare spending based on state income levels and plays a crucial role in the financing and sustainability of the Medicaid program. Currently, there is no cap to the FMAP which ensures that those who need Medicaid are able to obtain the support it provides.  


Impact on Medicaid in the Proposed Budget Reconciliation  

 

Any budget reconciliation or other legislation that includes cuts to Medicaid, imposing work requirements, or restricts funding or eligibility funding would have a devastating impact on our communities, including the robust supports and services that CLASS provides: In Home and Community Services for Those with Disabilities | CLASS Community Pittsburgh. These programs provide individuals with the skills and connections necessary for independent living in their homes and in their communities. Without adequate funding, many may struggle to maintain their daily activities and community participation, ultimately harming their health and wellbeing, most likely leading to institutionalization.


99% of CLASS’s funding comes from Medicaid. Through our various programs, we serve approximately 380 individuals, some receiving round-the-clock care. This number does not include the individuals served through our Camp and Retreats, our partnership with Tech Owl and our Information and Outreach services, which reach thousands in the greater Pittsburgh region.  CLASS also employs close to 275 individuals. If reimbursement rates are cut, we will lose these employees. These dedicated individuals deserve to be paid a living wage, and we would not be able to provide that.


Additionally, the introduction of work requirements to keep and be eligible for Medicaid poses a serious threat to individuals with disabilities and their family caregivers. Many cannot work due to health challenges or accessibility issues, and imposing such requirements risks their access to vital healthcare. This could also burden family caregivers, who play a crucial role in supporting their loved ones, leading to increased stress and reduced quality of life for both caregivers and those they assist. In fact, many individuals on Medicaid rely on Medicaid transportation benefits to get to work, school, doctor’s appointments and more. Moreover, Medicaid cuts would affect our direct support workforce, which is vital for delivering quality care. Reduced funding could lead to lower wages and fewer training opportunities, resulting in high turnover rates and a shortage of qualified caregivers. A stable workforce is essential for maintaining the quality of services provided to individuals with disabilities.


Ultimately, cuts to Medicaid would hinder CLASS’s ability to innovate and adapt to the evolving needs of the disability community. Medicaid is a critical safety net that enables individuals to lead fulfilling lives. We urge you to advocate for the protection of Medicaid funding and oppose work requirements, as these changes would have far-reaching implications for individuals, families, and service providers.
