Inspiration

Imagine you're a patient who has spent three months tracking every flare-up, every sleepless night, every moment your knee gives out on the stairs. You walk into your pre-op appointment and hand the surgeon a paper form with ten checkbox questions. None of your story makes it into the OR. Meanwhile, the surgeon is mid-procedure and has a question about the implant in their hand and the rep who knows the answer isn't in the room that day. According to HIPAA Journal, 80% of serious medical errors result from miscommunication, contributing to nearly 2,000 preventable deaths and $1.7 billion in malpractice costs every year. GreenOR was built for both of those moments.

What it does

GreenOR is a two-sided mobile platform (with web view support) that transforms patient voices into surgical intelligence and puts an AI-powered assistant directly in the surgeon's field of view.

Patient side: A guided journaling feature lets patients log symptoms, pain levels, and condition updates in natural language before surgery. No need for complicated forms and no medical jargon.

Doctor side: An AI pipeline ingests that unstructured patient input and surfaces a clean, categorized patient profile ready for pre-op review; organized, actionable, and easy to reference in the OR.

In the OR: Via Meta glasses, surgeons get a hands-free, voice-activated AI assistant with full context on the patient profile and medical tool specifications and can answer questions in real time without breaking sterile field.

How we built it

We split the project into three parts so each could evolve independently: 1. A backend (Node.JS/Express) for all API logic and AI orchestration 2. A web dashboard (React) for provider-side review and interaction 3. A mobile app for patient (React Native + Expo) for symptom logging, education, and record submission - which includes a provider-side for quick reviews/questions.

This gave us one shared backend contract while supporting two different user experiences.

Challenges we ran into

Originally, our project was going to incorporate ElevenLabs because we thought it would be a perfect fit to use their service for the voice of our OR AI assistant. But due to unexpected news surrounding the company's involvement in this event, we had to pivot from this idea.

Accomplishments that we're proud of

  • End-to-end patient-to-surgeon data pipeline working within the hackathon window
  • A genuinely hands-free OR assistant experience via wearable glasses
  • A product that meaningfully addresses all three sponsor missions simultaneously

What we learned

Designing for high-stakes, real-world environments like operating rooms requires thinking beyond the screen. The constraints of sterile field, cognitive load, and time pressure pushed us toward a more intentional, minimal interface than we originally planned.

What's next for GreenOR

  • HIPAA compliance review and secure data architecture for real-world deployment
  • Expanded medical product knowledge base for the OR AI assistant
  • Integration with hospital EHR systems for seamless pre-op data flow
  • Pilot program with surgical teams for real-world feedback

How we completed each company challenge

Arthrex: We support Arthrex’s mission by turning fragmented patient reports, images, and updates into clear, clinically relevant summaries that help surgeons quickly understand each case before treatment. GreenOR improves decision support at the point of care by surfacing key risks, procedural context, and actionable insights in a format teams can use immediately.

Eightpoint: We met this challenge by building GreenOR to ingest unstructured patient input, including free-text notes, voice-derived content, and photos, then process it with AI into structured clinical summaries, risk signals, and actionable care context. The result is a workflow where messy real-world data becomes clear, usable information for both patients and providers.

FGCU: We promote environmental sustainability by replacing paper-based pre-op forms and medical records with a fully digital patient intake and record management system. GreenOR eliminates paper consumption in the surgical workflow while enabling seamless information sharing between patients and care teams.
