Thank you for supporting the AMMI Glass Recycling Project! This form will ask for general contact information as well as the location you plan on putting the crate. It is important that the contact you supply will be held responsible for the crate. A pick-up request form will be shared to the email you supply us with. We will come to the location provided to collect the glass and replace the crate with another one. Please provide accurate information to facilitate the collection process.
