Dr. Karim Shafi shares the following case study: Patient is a 61yo F with a complex past medical history notable for psoriatic arthritis and bilateral total hip arthroplasty complicated by bilateral chronic PJI, s/p explant and antibiotic spacer placement. She is on iv daptomycin. She presented with worsening bilateral upper extremity weakness and numbness and progressive loss of hand function. She uses a wheelchair at baseline due to her hips and poor balance. mJOA 11 On exam, she was globally weak in her bilateral upper extremities, with hyperreflexia and bilateral +Hoffmans, and bilateral inverted brachioradialis sign Cervical imaging was notable for multilevel degenerative spondylosis, spondylolisthesis at C3-C4, C4-5 with subtle motion, and congenital stenosis. MRI of her cervical spine was notable for multilevel severe central canal and foraminal stenosis with cord signal change. Her CRP remained elevated >40 despite Abx therapy. She is planned for multiple further revision hip surgeries. In this medically complex case, I elected to perform a laminoplasty given her neutral alignment and favorable K-line, leaving laminectomy and fusion as a bail out option should she develop an infection in the setting of her PJI. Although her segmental instability was concerning to me, a 2016 retrospective cohort study (Spine) found comparable clinical outcomes in patients with or without spondylolisthesis who underwent laminoplasty at 2 years postop: https://lnkd.in/gfY4RNE2 At 9 months postop, patient has had preserved cervical alignment and improvement in her hand/upper extremity symptoms; her walking remains limited. Postop MRI demonstrates marked improvement in her cervical stenosis.
