PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

Five federal university hospitals (HUs) will integrate the National Network of Smart Hospitals and Services of the Unified Health System (SUS). The announcement of the network took place at the Planalto Palace in Brasília (DF), this Wednesday (7), with the presence of the President of the Republic, Luiz Inácio Lula da Silva; the Minister of Education, Camilo Santana; the Minister of Health, Alexandre Padilha; the President of the BRICS Bank, Dilma Rousseff; and the President of the Brazilian Hospital Services Company (Ebserh), Arthur Chioro. The University Hospitals are managed by Ebserh, linked to the Ministry of Education (MEC).

The SUS modernization initiative will have R$ 4.8 billion in investments, through a partnership with the New Development Bank (NDB), of the BRICS. The network aims to guarantee health infrastructure with advanced technologies in the five regions of the country. The multicenter Titan Trial project investigates a method to treat unilateral spatial neglect, a sequela of stroke that affects the perception of the opposite side of the affected cerebral hemisphere.

A clinical study involving several centers will test an innovative method to treat a disabling sequela of stroke: unilateral spatial neglect (USN), a condition that affects the perception of one side of space and hinders functional rehabilitation. Called the Titan Trial, the project investigates the combination of two non-invasive neuromodulation techniques – transcranial direct current stimulation (tDCS) and theta burst (TB) transcranial magnetic stimulation – with task-based physiotherapy sessions.

NEU causes a person to forget or not perceive one side of their body or environment. For example, they may only eat what is on the right side of the plate; collide with objects or people on the left side; comb, shave, or apply makeup only to the right side of their head; not hear when someone calls from the left side, etc. It is not a motor or visual deficit, but a perceptual deficit, which can continue even if the affected individual recovers strength and movement.

“This new initiative is a derivation of our research line in post-stroke neurorehabilitation, supported through Regular Grants from FAPESP,” says neurologist Rodrigo Bazan, professor at the Botucatu Faculty of Medicine of the São Paulo State University (FMB-Unesp) and coordinator of the study. Anvisa (Brazilian Health Regulatory Agency) has authorized the consumption and distribution of a new drug to combat Alzheimer's. Named Leqembi, the drug was officially registered on December 22. There is still no forecast for when sales will begin.

Anvisa approves clinical research on polylaminine, a substance that can regenerate spinal cord injuries
The medication is indicated for the treatment of adult patients diagnosed with early-stage Alzheimer's disease, already presenting mild cognitive impairment and dementia. It will be marketed in ampoules and is indicated for intravenous administration every two weeks.

How it works
Leqembi serves to reduce beta-amyloid plaques in the brain, since the concentration of these protein particles in the region is one of the determining factors for the onset of the disease.

Its effectiveness was evaluated using the CDR-SB scale, which measures degrees of Alzheimer's in patients. In the study, 1,975 people with early-stage Alzheimer's received Laqembi or a placebo. For patients who took the drug, there was a decline in symptoms after 18 months and less progression of dementia than those who received the placebo (1.22 versus 1.76 CDR-SB points).

Test for the ApoE4 gene
The test to identify the ApoE4 gene, the main genetic variant that leads to the risk of Alzheimer's, should be performed before starting treatment.

This improvement was seen in patients with only one or no copies of the ApoE4 gene.

The São Lucas Hospital of PUCRS (HSL PUCRS) will be responsible for coordinating an unprecedented clinical study in Brazil, which seeks to extend the application of the dengue vaccine to people aged 60 to 79, an age group for which there is still no authorized vaccine or conclusive data. The research, conducted in partnership with the Butantan Institute, will be carried out in five centers in the south of the country and will involve 997 volunteers. The study is scheduled to begin in the second week of January, and the Clinical Research Center at PUCRS University Hospital, responsible for coordination, is seeking volunteers.

People aged 60 to 79, evaluated according to specific inclusion and exclusion criteria, can participate in the study. In addition to the main group, volunteers are also needed to form a secondary group, made up of people aged 40 to 59, who will receive only the already approved vaccine without the use of a placebo. “It's a way to contribute to the research and still leave here already vaccinated,” explains Isabelli Guasso, manager of Clinical Studies in Infectious Diseases at HSL PUCRS.

Participants will be monitored for 12 months, with periodic visits, blood samples, and continuous health monitoring. The use of weight loss pens by elderly people requires caution so as not to accelerate functional decline, said Leonardo Oliva, president of the Brazilian Society of Geriatrics and Gerontology (SBGG), in an interview with Agência Brasil on Tuesday (6).

Without proper guidance, people aged 60 and over may be at greater immediate risk of adverse effects. These mainly include nausea and vomiting, as well as difficulty ingesting food and water, which can lead to dehydration and electrolyte imbalances, a potentially serious situation, said Oliva. In the medium term, malnutrition may also occur.

Another very important and significant risk in the elderly population is the loss of muscle mass when a person loses weight.

“About a third of the weight we lose with the use of these medications is muscle weight, lean mass. There is no way we can lose only fat. The body loses fat, but it also loses muscle.”

In the older population, this loss of muscle mass can mean loss of function, functionality, that is, the ability to perform daily activities.

“So, it is something very significant that may not even be recoverable.”

The scientific director of the Brazilian Society of Geriatrics and Gerontology (SBGG), Ivan Aprahamian, adds that the combined effect of decreased appetite, nausea, and rapid weight loss can precipitate geriatric syndromes such as sarcopenia and physical frailty.
