    Our insurance team is happy to submit accurate and fast PAs to help you secure coverage for your GLP-1 Medication, if possible.

    Cash pay for your medication is always an option!

    The cost of the program is $150 per month and this is due at the time of sign up. There are no additional fees for care or long-term contracts. The cost of your medications at the pharmacy is not included.
    To make your intake process as smooth as possible, please have these items ready in the device (computer or phone) that you will be using before you start…

    • Your Medical Insurance Information, Including Photos of the Front and Back of your Insurance Card. Bonus if you have a PBM (Pharmacy Benefits Manager) we need that information as well.
    • A photo of your driver’s license or passport.
    • A recent “Selfie” photograph that matches you to your ID.
    • If already on a GLP-1, a picture of your current GLP-1 medications with the pharmacy label containing your name OR the pharmacy or insurance records that show you have filled your prescriptions.
    • Any medical records you may have related to your weight management history and/or any documentation you can provide that show your weight history and trends. PDF or screenshots work!
    • A copy of any lab results that you may have from the past 12 months. (optional, but encouraged).
    • Your usual blood pressure and resting heart rate.
    • Your preferred pharmacy information (name, store number, address and phone number)!

    Your $150 monthly Subscription to the Vineyard Compassionate Care Package Includes: 

    • Medication Prescriptions: Access Brand Name GLP-1 or other weight and cardiometabolic health medications, if qualified and clinically appropriate. We handle the prior authorizations, insurance letters or appeals you may need and can help you navigate costs with manufacturer coupons, as needed. Prescriptions are sent to a pharmacy of your choice.
    • Easy to Use Vineyard Patient Portal: Request refills, talk to your team, access educational resources, book your doctor and dietitian visits and monitor your progress all from a secure web page.
    • Direct Physician Access: You have your own dedicated doctor you can message directly and actually talk to! You’ll never have to explain yourself to a care coordinator or wait days for your team to see your messages. Your doctor replies within 24 hours Monday through Friday and actually gets to know you as a whole person.
    • No One-Size-Fits-All Care: Your team will create a tailored plan for your long-term health that goes beyond just your weight. We address your sleep, blood pressure, cholesterol, and other factors affecting your health.  You doctor can order labs and prescribe more than just GLP-1s!
    • Keep Eating Food You Love: Your dedicated dietitian will partner with you to understand your unique nutritional needs and support you wherever you are in your journey. Chat anytime! Vistis are not required but the nutrition team is in your corner, always.
    • Private Community and Events: You’ll be able to interact with Dr. Spencer and your care team, and other members in an inclusive & well-moderated online private community. Attend live virtual events (support groups, nutrition classes, cooking demos & fitness classes, etc) and always feel supported.

    * Your Subscription does not include the cost of medication. While there are no refunds, there is no long term contract, you can cancel your subscription at any time.
