  • Most integration strategies are built on a flawed assumption: that once systems are connected, the job is mostly done. It isn't. The real cost shows up later. APIs change. Schemas drift.   • Building healthcare integrations has taught us something humbling. The hard part isn’t writing code. It’s dealing with reality. Reality looks like legacy systems that were never designed to connect. It looks like different interpretations of the same standard. It looks like “FHIR-compliant” APIs that still require careful translation. It looks like security reviews that rightfully ask tough questions. On paper, integration seems straightforward. In practice, it’s where complexity hides. We’ve learned that success in this space isn’t about clever engineering tricks. It’s about patience, rigor, and building infrastructure that assumes variation - not perfection. You can’t treat integrations as one-off projects in healthcare. The ecosystem is too interconnected, the data too sensitive, and the expectations too high. Every shortcut eventually becomes tomorrow’s bottleneck. At Propagator we think about integration as long-term infrastructure. Standardised data models. A unified API. Observability from day one.   • AI in healthcare is everywhere right now. New models, new copilots, new automation layers - all promising to transform care delivery, operations, and decision-making. The momentum is real, and so is the potential. But there’s a quieter issue that doesn’t get the same attention. Most AI initiatives in healthcare don’t struggle because the models are flawed. They struggle because the underlying data is fragmented. Information lives across EHRs, PMS’s, billing systems, CRMs, legacy databases, and bespoke platforms that were never designed to work together seamlessly. Standards like FHIR and HL7 help, but they don’t eliminate inconsistency or interpretation differences. Security and compliance requirements add another layer of complexity. When data flows are brittle, AI doesn’t scale. A proof of concept might work in a controlled environment, but production exposes the gaps - incomplete datasets, delayed synchronisation, inconsistent mappings, or governance concerns. Intelligence built on unstable infrastructure eventually hits a ceiling. This is the unglamorous part of digital health. Integration doesn’t make headlines. It doesn’t feature in conference keynotes. It often becomes a budget debate rather than a strategic priority. Yet integration is the foundation that determines whether AI delivers measurable value or remains an experiment.   • “Just use the API” is standard advice in healthcare integration. It’s also where a lot of projects quietly start to slow down. Healthcare APIs aren’t a clean abstraction layer. They’re a patchwork: - Different paradigms (REST, SOAP, GraphQL) - Incomplete, outdated, or contradictory documentation - Version changes that break working integrations - And plenty of legacy systems with no APIs at all Even when an API exists, it rarely reflects how the system is actually used. Important fields are missing. Meanings don’t line up. Standards move faster than real-world implementations. The result is predictable: teams write glue code, stack workarounds, and spend ongoing effort keeping brittle integrations alive. Integration becomes an engineering tax instead of a capability. Propagator removes that friction by taking on the complexity of APIs and legacy interfaces. Teams focus on what data needs to move, when, and why - not how to wrestle it out of each system.
