Disruption Summary
Impact: Between 4–18 August 2026, users on affected sites could experience issues with Momentum activities in courses and events. Users entitled to an application may not have had it load as expected, and users who weren't entitled received no explanation. Administrators were also unable to edit affected activities.
Cause: A code change deployed in June 2026 to improve validation and error messaging was inadvertently reverted for a single component during a later, unrelated merge, causing a mismatch between two parts of the same feature.
Resolution: The issue was identified and the correct code restored on 18 August 2026. The fix has been verified across both permitted and blocked user scenarios. No data was affected, and no migration or backfill was required.
