    Rare, real, and actionable. Today’s Reuters coverage of our new nference study highlights why large-scale observational evidence matter as GLP-1 therapies become part of routine care. The most important finding from the #AI-augmented curation of routine clinical notes is not simply that hair loss can occur. It is that weight loss alone may be an incomplete explanation. After matching patients for achieved weight loss, new-onset alopecia remained more frequent with tirzepatide than semaglutide, 4.24% versus 3.33% (RR: 1.27; P<0.001). Among women, rates were 5.44% versus 3.63%. Patients who developed alopecia were also enriched for thyroid and reproductive-endocrine conditions, while repeated laboratory measurements did not reveal broad depletion of commonly measured vitamins or minerals. For physicians, this means not considering hair loss as simply the price of weight loss alone. For consumers, it means hair loss is uncommon, not inevitable, and deserves individualized counseling and evaluation rather than automatic treatment discontinuation. This is why real-world evidence matters. It complements randomized clinical trials (RCTs) by revealing how benefits and risks differ across apparently similar therapies, the biology unique to each patient , and lived clinical trajectories across large cohorts of diverse patients.     For many patients considering GLP-1 therapy, one fear sits quietly behind the first prescription: "What happens if I cannot stay on this medication forever?" Today’s thoughtful NBC News coverage by Simar Bajaj gets to the heart of that uncertainty. Our real-world nference study suggests that stopping tirzepatide (Zepbound, Mounjaro) or semaglutide (Wegovy, Ozempic) does not automatically mean regaining all the weight. Most patients maintained their progress six months after their last documented GLP-1 prescription. Importantly, patients who maintained their weight loss were more likely to have received exercise counseling during treatment, suggesting that the habits and support built during GLP-1 therapy may matter long after the final dose. This study is not a promise that weight regain will never happen. It is something equally important: evidence that patients should not be told failure is inevitable. Real life is more complex than discontinuation within a gold-standard randomized clinical trial (RCT) protocol. Patients adapt. Physicians personalize. Biology differs. And real-world uncertainty deserves the best possible real-world science, beyond inferencesdrawn from a controlled setting. RCTs are the necessary first step in bringing a treatment to market, but they are not sufficient on their own to ensure that every patient receives the full benefit they deserve.     When does successful weight loss begin to become loss of physiologic reserve? Reuters highlighted the new nference study with peer-review ongoing that examined nearly 54,000 adults aged 65 years or older across three settings: #tirzepatide (#Zepbound, #Mounjaro) treatment, other antidiabetic medications, and #bariatric #surgery. Frailty-related conditions were uncommon overall. But when malnutrition, dehydration, appetite loss, or muscle wasting appeared, they marked a profound change in prognosis across all three pathways. Among #tirzepatide-treated adults, #malnutrition was associated with an approximately 25-fold higher risk of death compared with patients who did not develop malnutrition. The corresponding risks were approximately seven-fold with other antidiabetic medications and two-fold after bariatric surgery. Muscle wasting and dehydration followed a similar pattern. The signal was therefore not unique to one drug or one approach to weight loss. But among tirzepatide users, frailty phenotypes rose with greater weight loss, particularly beyond 20%, and often emerged only after several months of treatment. These observational findings do not establish that tirzepatide caused the adverse outcomes. They suggest something more immediately actionable: frailty may be the body’s early warning that intended weight loss is beginning to overlap with physiologic depletion. The future of obesity care cannot measure success only in pounds lost, BMI reduced, or glucose improved. Especially after age 65, we must also ask: Is strength being preserved? Is protein intake adequate? Is hydration maintained? Is independence protected? Powerful weight-loss interventions deserve equally powerful monitoring.
