Let's say the recipient has dormant CMV (IgG+)(R+). When they get immunosuppression/chemotherapy, the virus reactivates!
If the donor cells contain anti CMV immunity (IgG+ donor)(D+), this offers protection.
If the donor is CMV IgG - (D-), the risk is the highest!
