So far my thoughts on transitioning from academics to private practice!
1. My job is very similar. I see high acuity patients who need CART/BMT/acute leukemia+, clinical trials
2. When I ask for help from colleagues they are thrilled to see patient!
3. I enjoy the doctor lounge!

- Although I enjoyed my time off it does feel great to be practicing hematology again!
- Antibiotic Equations! Ceftriaxone = gram negative - pesudomonas - ESBLs Cefepime = Ceftriaxone + psuedomonas Metronidazole = anaerobes Pip/tazo (Zosyn) = Cefepime + metronidazole Meropenem/imipenem = Pip/tazo + ESBLs Ertapenem = meropenem - pseudomonas Q.E.D.
- Nice dermatology/heme pearl I saw today on rounds! Patient with AML getting 7+3 (cytarabine+daunorubicin). What’s causing this?
- How I still try to keep up to date with the literature! 1. Every week browse new articles in major journals for my field (Blood, JCO etc) 2. I still read/skim the UpToDate article for all new patients I see regarding their diagnosis 3. Follow key people on X 4. Call my friends
- DLBCL treatment has gotten more complicated Back in my day it was RCHOP if relapse salvage and ASCT
- It’s great to be practicing medicine again! And seeing patients in their 80s with AML still getting screening mammography.
- Deep thoughts on leaving academics… The amount of silly and redundant emails I receive has dramatically decreased since leaving academics. This is nice.
- Anaplastic large cell lymphoma represents many distinct clinical entities. This slide summarizes up nicely ALK+ = BV-CHP x 6 ALK- = BV-CHP x 6 +/- ASCT Breast implant ALCL = Remove implant Primary cutaneous ALCL = radiate or resect, more advanced then MTX or BV
- I have always enjoyed going to medical conferences but at this point in my career if I’m going to take time of work I can think of about a million other things I would rather do.

- Plerixafor = magic! We use it to help collect stem cells for BMT! Plerixafor binds CXCR4 receptor on HSCs and 🚫 interaction between CXCR4 and its ligand, CXCL12. CXCL12 tethers HSCs to bone marrow niche 🚫 its interaction with CXCR4 releases HSCs into the circulation!
- Given my Twitter account rebirth has failed I’m just gonna show pics of all the good food I get in the doctor lounge everyday. Today we had Jamaican jerk chicken and fried plantains. It was tasty, nice amount of spice.
