77 yo with NSTEMI, severe ICM (EF 20%), severe LFLG AS (AVA 0.8/MG 29), MV CAD (calcified diffusely diseased prox-mid RCA, dLM-mLAD). Turned down surgery. BAV first- improved MG to 20, Impella assisted single access PCI ✅TAVR outpatient next. #cardiotwitter @AtlantiCareNJ

Protected PCI with tMCS ➡️ single access 7F Command sheath always ➡️ IVUS no cross; pLM measured 4.5mm➡️ Roto LM-LAD, mLAD & LM-LCx➡️ Shockwave IVL LAD & LM, NC to Lcx ➡️ Stent mLAD / Mini-Crush for bifurcation PCI LM-LAD-LCx ➡️ FKBI ➡️ post IVUS ➡️ tMCS removed post procedure.

Protected PCI with tMCS ➡️ single access using 7F Command sheath always ➡️ IVUS ➡️ orbital atherectomy LM-LCx ➡️ Hydrophilic wire for LAD - 🎈 & Mini-Crush Technique for bifurcation PCI with LAD as SB ➡️ FKBI ➡️ POT ➡️ IVUS ➡️ tMCS removed post procedure. Home next day.
