    Delighted to announce that the @AtlantisTrial of low-dose titrated amitriptyline as a second-line treatment for #IBS in primary care is now published online in @TheLancet authors.elsevier.com/sd/article/S01… I will be presenting the results today at 1430hrs at #UEGWeek2023

    I speak as a consultant and an academic. I am not a BMA member but am FRCP. I think this undersells a serious issue as a generational one or a twitter storm. It is neither.

    Finally... A randomized trial showing proton pump inhibitors are safe when used for >53,000 years of follow-up. Hopefully this will receive as much attention as observational studies showing numerous potentially "spurious" deleterious consequences

    Medical students are graduating after years of arduous exams with huge (and unprecedented) amounts of debt, have seen massive erosion of their pay and working conditions, have to pay defence unions, exam fees, and fees to use a mandatory training record, etc.

    And in terms of the MDT of medicine as a whole, when asked about what unique skillset this profession brings to the MDT that a doctor doesn’t at the RCP EGM yesterday, the college leadership could provide no clear answer.

    Instead, what has happened is that this profession has been allowed to change its title, take on roles that younger doctors once fulfilled (instead of doing the less complex tasks that we were told they would be doing) and being paid more than younger doctors.

    They are now being set up as an alternative quasi-independent profession. Possibly with prescribing rights and ability to organise investigations that involve ionising radiation, without the same rigorous training. Anyone could have foreseen this would cause huge problems.

    They are losing training opportunities to a relatively new profession allied to medicine who were sold to doctors as being a solution to their levels of overwork.
