When your senior resident or attending tells you to order an imaging test, make sure you understand the purpose. If you don't, ask them! What are you hoping to answer with the study?
USMLE Step 1 will soon change to pass/fail
Predictions:
-residencies will start requiring Step 2CK prior to application
-more medical schools will shorten the preclinical curriculum and start clinical clerkships early in second year
-test prep industry will focus on Step 2
Failure to effectively communicate
A teen presented to the ED with chest pain and dyspnea. A CXR was reported below.
The patient/family were led to believe it was normal and were discharged home.
He died 3 days later.
Parents sued. Hospital just settled for $16 million. ๐งต

New guidelines for dizziness/vertigo in the ED:
1) Don't do noncontrast CT routinely!!!๐
2) If continuous + HINTS is central or equivocal, do MRI/MRA
3) If episodic spontaneous + concerned for TIA, do CTA or MRA head and neck (conditional recommendation)

A dramatic complication of invasive imaging: aneurysm re-rupture. Some angiographers wouldn't even save these images, much less blast them on the pages of @NEJM.
