Cache ActivityPub actor handles for cached posts

Summary

• cache ActivityPub actor acct values on incoming cached posts when available
• prefer cached acct / ap_actor_id lookups before host-based URL heuristics
• add a one-time migration for the latest 100 cached posts with ap_actor_id
• show only active, de-duplicated ActivityPub feeds in the author header
• use the subscription-aware local post query when checking ActivityPub subscription health
