Database tables not created

It has been reported that the tables for the plugin is not automatically created when

• using the plugin as MU
• duplicating a site using a duplicate tool
• network activiting

I'm 99.9% sure this is due to the register activation hook is not fired and that's when we create the tables etc.

Reported in email in support forum and I also noticed the MU issue on a site myself.

I got this error when using the plugin as MU plugin once:

Great, turns out it was that other 0.01 %. Most likely duplicate site plugins, or similar solution, that copied all core tables, including options table, but not custom tables. So simple history had options set with its db version. But.. no tables!

The solution was to catch db errors when doing the log() or overview_query() methods and then recreate the tables.
