Markdown whitespace formatting improvements

There was a bunch of inconsistent whitespace in the markdown documents. So I did some cleanup:

• remove spaces around anchor text
• add final newlines
• remove trailing whitespace
• remove extra space after name= in _specifications/base/*/specification.md

And while I was at it, I also noticed this too:

• fix incorrect anchor for Request: `textDocument/implementation` in _specifications/lsif/*/specification.md
