Right now it is possible to call QueryStatus::addPipelineExecutor() when
the executors_mutex already acquired, it is possible when the query was
cancelled via KILL QUERY.
Here I will show some traces from debugger from a real example, where
tons of ProcessList::insert() got deadlocked.
Let's look at the lock owner for one of the threads that was deadlocked
in ProcessList::insert():

How is this possible? The answer is fibers and query cancellation
routine. During cancellation of async queries it going into fibers again
and try to do this gracefully. However because of this during canceling
query it may call QueryStatus::addPipelineExecutor() from
QueryStatus::cancelQuery().

This commit adresses below leaksan find. I tried to reproduce this locally but 02802_clickhouse_disks_s3_copy.sh but couldn't. clickhouse-disks did not do anything useful (it would not even print logging), neither with a standard build nor with a leaksan build. Could not find further documentation of it or even what this tool is supposed to do, perhaps it is just for internal use. Also, - line numbers in the leaksan report were partially missing, - I am not really sure how Sha256HMACOpenSSLImpl::Calculate is calling into hmac_init (there must be some sort of static initialization somewhere but I did not find it), and - my fix is in a weird place due to other restrictions (see the commit in the aws-sdk-cpp contrib repo). The chance that this fix fixes the leak are low. If it doesn't work, add "# Tag no-asan" + a comment to 02802_clickhouse_disks_s3_copy.sh and don't worry further. …ed durable meta (triage #4) The top-severity item of the 2026-07-17 codex-review triage (§3.4): `scheduleMetaJob` swallows every exception from `writeCondemnedMeta` while the round commits the retired (hash, token) entry regardless. The per-hash meta is the writer's adopt gate (absent/Clean => same-token adoption without re-upload), so a swallowed condemn-marker write lets a writer landing in the [discovery-cut, deleteExact] window adopt the exact token a graduated entry then deletes — an exact-token delete of a body under a live committed edge (dangling manifest). The disaster-recovery rebuild wrote NO markers at all, making the hazard systematic after a rebuild. Fix (verifier option (a)): keep GC's async/advisory meta model everywhere EXCEPT the one edge that authorizes an irreversible delete — graduation to `delete_pending` now requires CONFIRMED durable `Condemned` evidence for the exact (hash, token): - both condemn sites (fresh zero-in-degree condemn + resurrect-supersede re-condemn) go through `scheduleCondemnMarkerWrite`, whose completion records the in-process (hash, token) confirmation when `writeCondemnedMeta` (now returning whether durable Condemned evidence exists) succeeds; a swallow records nothing; - the merge's graduation branch consults a `confirm_condemned_marker` gate: in-process confirmation, else ONE synchronous `loadMeta` re-check (per-hash `Condemned` is sufficient: an adopting writer must have observed non-Condemned earlier, its EDGE-BEFORE-OBSERVE edge landed before the meta turned Condemned, and the redelete only fires from a later fold whose cut sees that edge and spares); still unconfirmed => CARRY the entry (fail-safe delay, never a fail-open delete), count `CasGcCondemnMarkerUnconfirmedCarry`, and RETRY the marker write (liveness); - `delete_pending` rows persist the confirmation as a `marker_confirmed` bit (`CondemnedRow` flags bit 1, `mc` in the cas_run NDJSON codec; pre-release, no compat); - the rebuild publishes `writeCondemnedMeta` for every `zero_condemned` entry SYNCHRONOUSLY; per-entry failures leave the entry unconfirmed (carried by the gate) and never abort the administrative pass. The standing GC rule is untouched: the gate never throws — a 404/unreadable meta during the fold is missing evidence (carry), not a wedge. TLA+ gate: docs/superpowers/models/CaGcCondemnMarkerGate.tla (+ run_condemnmarker.sh). The pre-fix config (GateOnMarker=FALSE) reproduces the exact §3.4 counterexample (swallowed marker -> unconditional graduation -> same-token adopt in the cut window -> delete under a live committed edge violates NoDangle); the post-fix config passes, with graduation/adoption reachability checked non-vacuous. Tests: swallowed-marker carry + heal-liveness, canonical-schedule preservation with the durable bit, rebuild marker publication, rebuild swallowed-marker carry; CondemnedRow flags round-trip + unknown-flag fail-closed. Recovery no longer believes a listing. `ensureRefTableRecovered` now follows spec §4: read `_ckpt` ->
fetch the base snapshot by EXACT KEY (greatest of the hint and `_ckpt.checkpoint`) -> walk the tail
arithmetically from `base + 1` -> close every dead epoch below the live one with an in-band
`EpochSeal` at `{E, T+1}` -> contribute the chain link to `_ckpt` -> re-check the fence and install.
THE BLOCKER'S RECOVERY FACE. A `LIST` that omitted a key produced a table missing an ACKED
transaction and looked healthy. Ids are dense `1..T` within `(namespace, epoch)` (INV-1), so
completeness is now arithmetic: a hint omission costs one exact-key `GET` and changes no outcome,
while a 404 BELOW a confirmed same-epoch witness is a HOLE -- restarted while the restart budget
lasts (a racing cleanup is the innocent explanation), then `CORRUPTED_DATA`. Never "fold what we
have".
THE TRIO COMPLETES HERE. One `admitted_generation` is captured at entry and presented three times:
at every `slotOccupy`, at the `_ckpt` CAS, and -- NEW, the r9-5 #4 gap -- under the re-acquired
`state_mutex` immediately before `installRecoveryResult`. A recovery whose I/O window straddled a
fence bump now installs nothing; before this there was no such check at all.
THE SENTINEL SEAL IS RETIRED, and with it `RefTableSnapshot::sealed_from` (wire keys `sfe`/`sfs`),
`RecoveryResult::sealed_from`, `RecoveredRefTable::sealed_from` and the runtime field. The old seal
was a snapshot at a synthetic `{E-1, UINT64_MAX}`: it occupied no log key, so it fenced nothing. The
replacement is not a quieter fallback -- it is the store's own write-once create, at the exact key a
dying predecessor's in-flight PUT would have taken.
Also here:
- `RefTableState::applyOp` implements the `EpochSeal` apply (was `NOT_IMPLEMENTED`). Mandatory, not
incidental: the walk MINTS seals into the durable stream, so without it every sealed namespace
would be permanently unrecoverable on its next recovery. A seal carries no table content and
requires `Live` -- the read half of the same rule the minting side enforces.
- The self-remount CANCEL-OR-JOIN barrier (spec §3, codex finding 10):
`cancelRecoveriesAndAwaitQuiescence`, called from `tryRemountOnce` before the quiesce and the fence
re-arm. The walk polls cancellation, supersession and the generation at every I/O boundary.
- A POISONED table re-recovers on its next touch (task 3 ledgered this structural closure here): the
walk re-derives the stranded transaction from the durable log and the install clears the poison.
Gated on there being no wedge, whose durability is undecided by construction.
- `unclean_boundary_epoch` is deleted. Sealing is decided by `epoch < live_epoch` alone: the seal is
the chain link that makes a MISSING epoch detectable, and a chain that skips every cleanly-shut-down
epoch is not a chain. This is a deliberate behaviour change -- a clean boundary now seals too.
Test fallout, each stated rather than absorbed: `RefWriterRecoverySeal` (12 tests) is retired with the
mechanism it pinned and replaced by `CasRefRecoveryCasWalk` (18); `CasRemountTmat`'s clean-boundary
test now pins the new rule and that no synthetic seal snapshot is written; the recovery-retry suite
faults the seal's LOG key instead of a snapshot key; the codec/inspect/encoding-pin tests lose the
retired field, keeping the `UINT64_MAX`-round-trip property they were really protecting.

…thority, four latent test bugs fixed
test_cas_gc_sharded has been permanently skipped since it was written (MinIO OSS lacks the
conditional-DELETE support the CA mount capability probe requires), so this module has never
actually executed until now. Enabling it surfaced four independent, pre-existing, latent bugs
in the test itself -- none in production code:
1. Wrong S3 backend: swapped with_minio=True + a MinIO endpoint for with_rustfs=True + the
standard rustfs1:11121 endpoint/credentials, matching every other CAS integration test.
2. A Python str.format/ClickHouse-macro escaping bug: the live_table CREATE queries wrote
'{replica}' (single braces) instead of '{{replica}}' (double braces), so .format(STORAGE_POLICY)
raised KeyError('replica') on its very first query. The correctly-escaped form already existed
two functions below in the same file.
3. A post-restart race querying system.text_log right after node2.restart_clickhouse(kill=True):
the underlying table can take a moment to become queryable. Fixed with a keyword-free
readiness probe via the existing query_with_retry helper, run once before the real keyword
checks (not by retrying the real checks themselves -- retrying THOSE is self-defeating: a
failed attempt's own <Error> log line echoes the failing query text, which contains the search
keyword, and that line then lands in system.text_log itself, so a later successful attempt of
the SAME keyword query counts its own failed predecessors as matches. Verified directly: a red
run's "6 == 0" failure was exactly 3 retry attempts times 2 log lines each, all
"Unknown table 'system.text_log'" echoing the '%DANGLE%' query text; zero real dangle-shaped
log content anywhere in the run).
4. The completion-detection and shard-coverage assertions polled entirely wrong S3 keys:
"completion_seal" never existed anywhere in production (grep returns zero hits; the real name
is "fold_seal"), and the assumed path shape omitted the "attempt/<attempt>/" segment production
actually uses. Rewritten to resolve the ADOPTED (generation, attempt) pair from gc/state's own
text wire format (fields "sg"/"sa") -- the same two-hop lookup production's own
Gc/CasOrphanManifestSweep.cpp readAdoptedFoldSeal uses -- so a retry-created, never-adopted
attempt's own seal can never be mistaken for a second completion signal. Assertion C (shard
coverage) scans the whole gc/gen/ subtree rather than one (generation, attempt): a seal's
blob_target_runs carry a PARENT's runs forward as references, but the physical objects stay
under the (generation, attempt) prefix where they were originally written, so a late-soak
adopted attempt with no new deltas of its own can have an empty prefix even though earlier
attempts wrote plenty.
Six-run arc: #1 (minio->rustfs swap only) red on the brace bug; #2 red on the text_log race;
#3 red went past both into the real key-layout bug (branch confirmed: wrong key name AND wrong
