fix(runtime): enforce swarm failure boundaries

Summary

• validate declared tool parameters in ToolRuntime before invoking implementations
• advertise implementation agents only when the current project supports Git worktrees
• classify all-failed agent swarms as failed across result decoding, events, and telemetry
