    Is the dark secret of US healthcare that doctors just make too much money? Or is it the PBMs? The insurers make a few percent, the providers are on the brink of bankruptcy, drug spend is ~15% of total healthcare spend. Where else is the money going?

    Once again, insurance companies have a 3% profit margin. They physically do not have the money to approve more claims. The only way for them to approve more claims would be to raise insurance premiums.
