Grateful to @BloodJournal, co-authors, and @SWOG for making this #downwithdex piece possible! Dex 40mg weekly in #MMsm induction: 1️⃣ Not practical even for trial-enrolled patients 2️⃣ Does not improve PFS vs ⬇️ dex Twitter 🧵 once I’m back from #IMS24! ashpublications.org/blood/article/…

As an avid @xkcdComic reader for over a decade, my only regret is that it took me this long to meme-ify one of his excellent comics. Exciting time to be a budding academic in the myeloma #mmsm field!

1/ Excited to be joining the myeloma #MMsm group at @UWMedicine / @fredhutch this summer as Assistant Professor. Thrilled to be continuing my research in #digitalhealth, #patientexperience, & #TcellRx. This 60-hr drive has taken 33+ years... but I can't wait for what's to come!

1/ Sobering but important work in @NEJM by @MScordoMD @szusmani et al: fatal case of enteric T-cell lymphoma following cilta-cel (#MMsm CAR-T) CAR integration into the TP53 locus. One of few papers that informs clinical discussions the moment it comes out. My two takeaways:

1/ So excited to see this out! nature.com/articles/s4140… In myeloma, trials continue to use 2x weekly bortezomib because of a belief that standard of care (SOC) is only based on older trials. In reality, the SOC is how typical physicians would approach #MMsm care - so we asked!

As myeloma doctors would say, I’m excited that our family has moved from triplets to quadruplets when it comes to car seating arrangements! 👨 👩 👦👧 🙏 my colleagues for covering my panel. Our daughter was born 5 days ago and seems to already love point-counterpoint debates!

Peer review is too often a thankless job (beyond perfunctory “We thank the referee for their comments”). So I’m beyond touched to have gotten this beautiful gift from @JCOCCI_ASCO @ASCO today. The photo does not do justice to its iridescence! Going to have to get this insured 🥰

I am so sorry to hear that Yara! Thoughts on publicly naming the airline and we will unleash an army of shame on them and maybe some media attention?

Excited to start seeing patients at @UWMedicine / @SeattleCCA - and even more excited to have my own office for 1st time! Key parts: tracker so people know they can come in, my name so they know whom they’re meeting, … and an #MMsm meme to really know whom they’re meeting 🤣

(Non-medical post) My son’s first time seeing legit snow, and my heart could not be more full! Even if he decided to wear his Seattle rain boots instead of the snow boots we got him 🤣🤣 happy early Valentine’s Day to all!

👏 I saw an interesting anecdote and just looked up the reference. Literally before I was born, an oncologist very succinctly and elegantly summarized how we offer hope to our patients living with cancer... these words are still true today! pubmed.ncbi.nlm.nih.gov/18750361/

With dex about to trend on Twitter and with new interns starting, two tips: 1) “Harry Potter, MD when he was twenty.” H-P-M-D = 20:5:4:1 ratio of hydrocort/pred/methylpred/dex. So 6mg daily dex for COVID19 = 30mg-ish pred. Not much! 2) Dex is 1:1 IV:PO unlike pred/methylpred 🎉
