    Taskification and substitution in the NHS - a hypothetical case of how splitting up pt care into "tasks" misses the diagnosis & costs more: A recently married 45 yr old Pt develops bloody diarrhoea over 4 weeks. "The protocol" says they need a blood test, stool MCS & FIT test 1/

    Way back in 2015 when Twitter/X and social media was not as big a thing as it is now, back in 2015, there were lots of concerns how #PAs where being pushed forward by @RCPhysicians whilst junior doctor strikes were on the horizon 1/x

    Please be kind to the good, nice #PAs who have been sold a lie. Please hold those who made this disaster happen, despite warnings, accountable. I am sadly retired now. Keep fighting for your patients, even if they try to discredit you as 'troublemakers'

    The @RCPhysicians has been run into the ground. No lessons have been learnt. The leadership is out of touch. This is #NotMyRCP and this decision against @parthaskar must be rescinded immediately

    But this is what, in my opinion, is what is happening in the NHS. Better at diagnosis? Nope. Better care? Nope. More unnecessary tests? Yes. I know it sounds old fashioned but a good history & examination by fully medically trained workforce is what is needed. Not gimmicks end/

    The longer this goes on @gmcuk, the more embarrassing it is & the more the more it shows you don't follow your own guidance regarding candour, transparency & probity. It demonstrates you don't want to listen. @doctor_oxford's reach is enormous & this is the "Streisand effect" 1/2

    If you don't feel upset and angry that many doctors are going to be unemployed, whilst cheaper, less trained staff are brought in to replace them, I question why? Maybe the powers that be have already won, or maybe you never cared in the first place? Either way it is very sad.

    I sat opposite @DacreJane in 2015 (in a Hilton Warwick) & told her the term "associates" would cause confusion. I had already told her it was a bad idea in a RCPL trainees meeting. She did not listen & facilitated its progression. Will there be a sorry now from her & others?

    Note: In today's NHS the outcome of this case at a morbidity (nearly mortality) MDT would be "followed protocol", all good. That is how messed up everything is.

    I don't know how the UK has avoided a mass walk out by Drs? I am being serious. How much worse does it have to get? "Leaders" have let you down SO badly over the course of at least 10 years *whilst* promoting your substitution and accepting worse T&Cs. When is #EnoughIsEnough?

    The senior Consultants rolling their eyes at potential @BMAResidents strikes (if Gov fails to negotiate a fair deal) are exactly the reason why pay was allowed to degrade so far that Drs are on as little as £17/hr - the culture of "shutting up and putting" up is over #ThankGod

    It also struck me there was no insight into how a doctor has done 5-6 med school, 2 years FY, 2-3 CMT and then ST posts with ARCPs, exams, hurdles galore, to lead ward rounds, do procedures etc, but somehow a #PA could achieve the same level of competency after any science 16/x

    BUT, crucially THEY did not listen to us ........ and 10 years on a group of Anon accounts such as @Xeon4f145d96s1 @ExplosiveEnema @Dr_Done_ and others and the irrepressible @DrEilidhMaria @mattster have pushed this matter to the fore in a way that was not possible in 2015 25/x
